First Session. This is an opportunity for both of us to get to know more about each other. We discuss what is currently bringing you into counseling, what has been useful to you in the past, and what you hope to achieve here. I begin collecting a health and family history which continues throughout our work together. This is also your opportunity to determine if I am a good fit for you.
Confidentiality. All of the information we discuss is confidential. If requested I can release information only through procedures which are consistent with the law and professional ethics. Your records may only be released with your signed consent or by court order. In accordance with State law, I am required to report any threat of harm to yourself, another person, or any incidence of child or elder abuse.
Availability. My practice is not emergency based. I am often not immediately available by phone but will return your call as soon as I am able. If you think that you are currently in a situation which requires immediate assistance, please call 911.
Payment. My fee for a 45 minute session is $110. I accept cash, checks, and credit cards. If you are using your insurance benefits this fee varies.
Services. Psychotherapy is a voluntary, relationship-based intervention, the results of which are not guaranteed. At any time you are free to discontinue treatment or seek other services.
Cancellation policy. Like most medical providers I require 24 hours notice for any appointment cancellation, but appreciate at least 48 hours, unless you are sick or have an emergency. This is because I have a limited number of appointment slots and can to fill the slot on short notice.
Insurance. Insurance coverage can be confusing. Please contact your insurance company to determine your mental health benefits. It is much easier for you to obtain this information about your policy, then for me to access. I am an in-network provider for BCBS PPO. Some other insurance providers may cover my fee if you have "out of network" benefits.
